That was a quick whirlwind overview of Glass, healthcare, workflow, and innovation.

So now I’m going to connect Glass to the projector. Demo a couple things, such as interacting with the touchpad and a speech command. Maybe answer my email hands free.

Then I’m going to ask for a volunteer, perhaps more than one volunteer if we have the time. Because in spite of my blather, the best way to understand Glass is by putting it on an playing around with it.

Slides 32-33: Personal Palliative Care Google Glass Idea

Now we get to my personal favorite. The idea is based on watching my father and father-in-law during a long period of convalesce during which they were weak and bed ridden. I’m bringing it up here more or less to possibly network with other possibly interested in the same. The essential insight is this. In each case their worlds become smaller and smaller. As they become weaker and weaker, the effort both physical and mental, required to do such things as even pick up a smartphone let alone a tablet become too much.

There’s this wonderful phrase in usability engineering “cognitive friction.” If you think about pushing something on a surface, there’s resistance due to friction. As you get weaker and weaker, eventually you can no longer move the object. Using computers also requires effort, and cognitive friction is a metaphor resistance to their successful use. Earlier I spoke of minimizing time between intention and action. That’s also minimizing cognitive friction. Even a small amount of cognitive friction can completely stop you if you are weak or distracted enough by pain or drugs.

In this slide we see the Glass configuration screen, that allows you to set the angle above which the Glass micro display automatically turns on. I’ve experimented and you can get the interaction between this set angle and your own position so that virtually imperceptible movements of your head can turn Glass on and off. So I imagining seeing Glass to, say, pull photos of the family from an online service, but to only do so when you head is moved to a certain position, or perhaps to a certain position in conduction with a head gesture.

Relative to patient experience, which I’ve also already briefly covered, Glass can help educate, help navigate hospitals (I’ve seen a really cool indoor navigation program). After I’ve received bad news, I’m not absorbing info well, I’d like to be able to leave a medical office with a video of what was said, perhaps even annotated with links to additional material.

Slide 30: Google Glass Visual, Hearing, Cognitive, Emotional Assistive Tech

Relative to assistive tech, I’ve broken it into four categories: Vision, Hearing, Cognition, and Emotion. Blind or low-vision folks may use Glass to recognize objects. Deaf may use Glass to transcribe conversation. Facial recognition and emotion detection programs may give little nudges to the forgetful. I can even imagine Glass apps that monitor excitability and emotional state and pop in no and again with a suggestion or offer or relevant action.

Google Glass and medical education uses includes just-in-time training. One of my twitter followers suggested integrating Glass into nurse training simulations. Remote training of course. Clincal simulation, again, and collaborative learning. Here I can imagine using Google Help Outs to allow experts to look through your eyes and help guide and mold your behavior. When I was a medical student I’d love to have worn glass in anatomy and been able to see what anyone else was seeing, or to have an expert look through my eyes and explain what I’m seeing.

Of emergency medicine use, I’ve already spoken of some of these on the earlier introductory slide: video from EMT to MD, just-in-time decision support, looking up information when time is tight and your hands are literally full, as well as documenting what is done and using that to improve emergency response.

Slide 26-27: Google Glass Surgery, Emergency, Med Ed, Assistive Tech, Patient Experience, Palliative Care

This slide is not based on a quantitative analysis. However, I ‘ve tweeted over 2000 tweets with the #GoogleGlass hashtag, and most of those were related to medical and health application for Glass. So I have a good idea, at least relative to tweets about Glass, what’s being discussed a lot. The column on the left is m y list of most tweeted about applications for Glass. The column on the right is my list of personal favorite areas.

Relative to surgery, the hands-free, not have to break sterile field aspect, aspect of Glass is especially attractive. There’s been a steady stream of tweets about the first this or first that use of Glass during surgery while streaming video, sometimes to another expert, sometimes to students, sometimes even live youtube video.

Glass for early responders is a favorite too. Whether you’re an EMR in the back of an ambulance or a nurse first to bed side when a specialist is five minutes away the idea if distributed synchronized team situational aware is a really popular vision of the future.

Again, in education, I remember how long you had to wait to just to get a glimpse of that beating heart and the proceeding fix. Now student will almost literally be able to see through the eyes of the most expert surgeon. My favorite app, though, I’ve not seen, but sure must exist in the sparkle of someone’s eye, is just-in-time learning and memorizing in all those interstitial moments that are otherwise unproductive. I had to memorize about 20,000 new terms the first year of medical school. A large reading vocabulary is only about 50,000 max. I’d have loved a omnipresent flashcard app to help me memorize all that stuff.

The next three are my person favorite areas. There’s assistive tech, using Glass to compensate for visual, hearing and other disabilities. There’s patient experience, both using Glass to collect data on what its like to be a patient and use that to improve that experience. As well as giving Glass to patients to help them navigate hospitals, stay in touch, are review educational material, both by video or perhaps even interactively.

Finally, I’ve a real personal favorite. I think Glass might be use to comfort and help the weak and bed ridden, through control of home automation, communication to loved ones, and least effort-based review of photos and videos from their life.

Slide 24-25: Google Glass User Interface Guidelines

This next slide is a distillation of a bunch of user interface guidelines pages on the Glass website laying out what should go where on the screen. These outside rectangles are the gutter that should be empty. Sometimes when you ‘re weating Glass one of the gutters may be fuzzy due to momentary mis-position of the headset. The Twitter account and URL are just there to document where the slide came from, as it get copied across the web. It’s not an app, its just a image to be used by developers. This next prototype isn’t so much about not getting in the way, but it is about making sure you don’t overfill a single screen. I use the example of the Snellen Eye Chart. In my experience, about five percent of folks trying on Glass for the first time have trouble seeing the display. First there is the question of which eye may be dominant. Then there is whether they wear glasses or not.

If you need glasses to see a 25 inch diagonal display 8 feet away, then you need to look through your eye glass to see the smaller fonts, down to 26 pixels in font size. So I created this mockup of Glass Eye Chart. When someone puts on Glass I ask if all the text is legible, or if not, what is the smallest font that is legible. This is useful for “seeing” if Glass is for you. I can imagine it might be useful for developer. Or, swiping from this card to the next card might lead to a map of nearby optometrists and eyeglass shops catering to glass.

Slide 22: Design For Google Glass: Keep It Out Of The Way!

If you go to on Twitter, it simply says “Getting technology out of the way”. And, it you think about it, given that the Glass micro display is only 640 by 360 pixels, with a minimum pixel font of 26, Glass can only display about one percent of what a 1600 by 1200 display can display. Add in the idea that glass is off most of the time, only becomes proactive when it is certain, by some means, that you need a certain tidbit of info or action initiation option. Therefore all of Google’s Glass user guidelines are about respecting these constraints. In fact the first “Design for Glass” are really an umbrella restatement of the next three admonishments. Don’t get in the way, keep it timely, AKA relevant, and don’t do anything unexpected. I think these are all really different ways of saying the same thing: offer all and only relevant info and options moment to moment. If only all EHRs and health information systems religiously followed these guidelines!
